Information for referrers

                      Thank you for the referral of your patient to Maxwell & Moore Psychology.               

Please kindly prepare a referral letter for your patient.  The referral letter can be to a specific psychologist or a general referral to Maxwell & Moore Psychology.

Please note that the MBS Better Access initiative stipulates that a valid referral letter should include the following:

• Date
• Patient name
• Patient symptoms or diagnosis
• Number of sessions (course of treatment - maximum 6 sessions per referral letter)
• Signature of referrer
• Provider number of referrer

Please fax the signed referral to 07 3740 1290, send via medical objects or email admin@maxwellandmoore.com. The referral must be signed to meet Medicare requirements.

Please note, we do not accept urgent referrals as we may be unable to offer an immediate appointment. If your patient requires urgent mental health care, please refer them to MH CALL on 1300 642 255.

Please ensure that your patient is aware that we are a private practice and we do not bulk bill.

Our practice values strong collaboration and communication with our referrers. We welcome contact from our referrers at any time to discuss client progress/treatment. We will always communicate with you at the following times:

• Maxwell & Moore Psychology will advise you once your patient has attended their first scheduled session, and advise of the plan for psychological treatment moving forward.

• Following your patients attendance at their 6th session, we will write to you to share a progress update and recommend any additional course of treatment for your consideration (if applicable).

• Following your patients attendance at their 10th session, we will again write to you with a treatment update.

​​

• We will then write to you at the end of that course of psychological intervention.
